| Aims | The service aims to move people along a pathway of rehabilitation and recovery towards independance and to promote social inclusion. |
| For Whom | Adults with a mental health need living in the Redcar & Cleveland area |
| Services | Creative Support, Redcar floating support works in partnership with social services and supporting people, providing both housing related support and care related support to people with mental health needs living in their own homes, community setting or supported tenancies. The service can be delivered over a shorter or longer period of time (up to 2 years) to prevent crisis, to support people to maintain their independance, help with resettling people into new homes or after a period of time spent in residential care, rehabilitation or other supported accommodation. |
