| Aims | Support independent living and social care support for people whose needs are met through care coordination arrangements (CPA) and support for people with mental health needs who are not on CPA. |
| For Whom | For people over 18 with a mental health need and who live in the local area. |
| Services | Open access drop in or activity (one of 3 places on different days in East Cleveland). Warm and welcoming places to lessen isolation by building up confidence, encouraging independence and focusing on your strengths. Information that promotes informed choice and encourages opportunities in the wider community. A quarterly reviewed programme. One to one support, access to telephone, fax, internet, I.T. and other materials/resources. |
