Emergency Duty Team

Contact in the event of an emergency, outside of office hours, at weekends and on public holidays.  We respond to situations involving child protectin, child care, mental health and other adult service matters.

 

The Community Mental Health Team can be contacted directly on 01642 230542.

Community Support Team

There needs to be a referral and a multi-disciplinary assessment. The team supports people to stay in the community by helping them to learn new skills. Examples are hygiene and personal care, shopping and cooking,budgeting and finance, transport skills and training. Support and information is given to help people take their medicine and to keep appointments.

A person will receive a multi-disciplinary assessment to assess whether they need the care and support offered by a particular residential care home.

Access Team

All requests for services are made through the Access Team.

(at other times contact the Emergency Duty Team number above).

An Access Worker will make an initial assessment of your needs based on the information you give.  This could include advice and information on services for example; occupational therapy, meals at home, care at home, appliances to help with daily living, support for people with sensory support needs, respite care (support for carers), family support services, safeguarding children, early years and fostering and adoption services.  Sometimes the Access Worker will refer you to another service able to help you.   Simple enquiries can also be dealt with by letter or a telephone call.
